If you are a BROOKE COUNTY businesses and are in NEED of PPE for your EMPLOYEES.
Please fill out the attached form and fax it into our office 304-737-3689.
The Brooke County Health Department can NOT guarantee requests to be filled as this DOES NOT come from our department.
We encourage you to also look for alternative ways in receiving PPE as this is NOT a guarantee to you nor your employees.
